Patients:

If you provided your Primary Care Provider (PCP) during check-in to Emergent Care, your records will be automatically transmitted to their office, provided your PCP is within the region and in our Medical Directory.

If you had services completed in our outpatient areas, the ordering provider listed on your prescription will automatically receive the results of your test.

Please allow us up to 48 hours to prepare your medical records. 

Records can be obtained by forwarding a signed Release for Information Form to MMECoffice@saratogahospital.org or mail to 6 Medical Park Drive, Suite 100, Malta NY 12020.
